PAYMENT OPTIONS:
Cash, and credit cards are accepted at time of service.
Some treatments require several visits.
With advanced arrangements, you may pay a portion of the total fee
at each visit with payment in full on the last visit.
CARE CREDIT: As a service to our patients, we are pleased to
offer Care Credit. This payment plan offers two convenient payment
options to meet every financial need, including interest free and
low interest options. Applying is easy. Apply now or ask the front
desk to assist you.
DENTAL INSURANCE COVERAGE:
Dental insurance plans cover between 50% - 100% of the cost
depending on treatment and plan in effect. As a courtesy we will file
claims for you and ask for you to pay your estimated co-payment. Please
understand that this is only an estimate and is based on the information
available to us.
Our office will accept assignment of benefits under the
following conditions:
The following information MUST be provided at the first
visit for verification of insurance benefits: Subscriber birth date and
social security number, insurance carrier billing address, phone number
and group number. After verification of insurance benefits, we will
accept assignment of benefits as partial payment of the total charge.
You will be asked to pay your deductible and estimated co-payment. It is
your responsibility to follow up with your insurance company if you do
not see a payment from them within 30 days.
THE IN HOUSE ADVANTAGE SAVINGS PLANAs a service to our patients without dental insurance, or even those
with poor reimbursement in a tough economy, we are pleased to offer this
great service. For a low cost annual fee, you will receive your
diagnostic checkup exams, needed x-rays, cleanings and fluoride
treatment for the year. In addition, you will receive a
substantial discount off of any treatment that you may need and even for cosmetic procedures that you may want! The best
part is, there is no annual maximum on getting the discount.
We have many patients enrolled who have literally saved thousands over
the years and allowed them and their families to get the dental care

PATIENT REFERRAL PROGRAM
At Dr. Owen Waldman's office, we appreciate when our wonderful patients refer
friends and family and we love to reward them as a result. Any
existing patient, who refers a new patient to our office, gets a $50
credit for each person they refer on their account. In
addition, each new patient that they referred, will also get a $50
credit on their account, when they come in for their appointment.
It's a win-win for everyone! We love that you are helping us grow our
